Cup to Disc Ratio in 30 Seconds

The cup to disc ratio (C/D ratio) compares the diameter of the optic cup to the diameter of the entire optic disc, most often measured vertically. It provides a rough estimate of remaining neuroretinal rim tissue and serves as a quick screening marker for glaucomatous optic nerve damage. On NBEO and OKAP, it is tested less as an isolated number and more as one clue within a broader structural picture.

What You Are Actually Measuring

The optic cup is the central depression of the optic nerve head, while the surrounding neuroretinal rim contains the retinal ganglion cell axons. The vertical C/D ratio is the most clinically relevant measurement because glaucomatous damage preferentially affects the superior and inferior poles of the disc, enlarging the cup vertically before horizontally. A large cup by itself does not confirm glaucoma. Disc size varies naturally between individuals, so a large disc physiologically has a larger cup without any pathology. Always judge the cup in the context of disc size: large discs normally have large cups, while small discs can harbor significant glaucoma despite seemingly normal C/D ratios. The diagnosis depends far more on rim quality, adherence to the ISNT rule, presence of disc hemorrhages and correlation with visual field and OCT RNFL findings than on the raw ratio alone.

  • Large discs normally have large cups and small discs can harbor significant glaucoma with a seemingly normal C/D ratio. Always interpret the ratio in the context of disc size.
  • The ISNT rule states normal rim thickness follows inferior greater than superior greater than nasal greater than temporal. Violation of this pattern raises suspicion for glaucomatous thinning even if the overall C/D ratio looks unremarkable.
  • Inter-eye C/D asymmetry of 0.2 or more is a stronger red flag than any single absolute ratio, since disc size is usually fairly symmetric between fellow eyes.
  • Progressive enlargement of the optic cup on serial examinations is more concerning than a single large cup measurement.

The Classic Trap

The most common exam mistake is treating the C/D ratio as a standalone diagnostic cutoff: assuming anything above 0.6 is glaucoma, or that a normal ratio rules glaucoma out. Physiologic cupping explains why this fails: a large optic disc naturally contains a large cup without disease. Conversely, a small disc with early glaucomatous damage may still show a normal C/D ratio because there is less room for the cup to enlarge. The rim's shape, ISNT compliance, hemorrhages and functional correlation with visual fields and OCT drive the real diagnosis, not the ratio in isolation.

Clinical Pearl

Inter-eye asymmetry is often more diagnostically useful than any absolute C/D value, because optic disc size is usually fairly symmetric between fellow eyes. A C/D difference of 0.2 or more between eyes suggests one nerve has undergone asymmetric damage, even if both individual ratios would look unremarkable when viewed alone.
