Cover Test vs Alternate Cover Test
Use the cover-uncover test first: cover one eye and watch the uncovered eye for movement, which detects a manifest deviation or tropia. The alternate cover test alternates occlusion before fusion can be re-established, dissociates binocular fusion and reveals the total deviation including any phoria. The alternate test cannot determine whether the deviation was manifest before dissociation, which is why sequence matters. Always perform the cover-uncover test before the alternate cover test in any strabismus evaluation.
Reading the Movement
- Inward movement: the uncovered eye was exotropic and moves inward to fixate. Exam relevance: this indicates exotropia of the moving eye.
- Outward movement: the uncovered eye was esotropic and moves outward to fixate. Exam relevance: this indicates esotropia of the moving eye.
- Upward movement: the uncovered eye was hypotropic and moves upward to fixate. Exam relevance: name the vertical tropia from the moving eye.
- Downward movement: the uncovered eye was hypertropic and moves downward to fixate. Exam relevance: name the vertical tropia from the moving eye.
- No movement: no manifest deviation is demonstrated. Exam relevance: consider orthotropia, a well-controlled phoria, or a very small tropia below detection threshold.
4 Facts Exams Always Ask
- The cover-uncover test detects tropias. It does not reveal a phoria when fusion remains intact.
- The alternate cover test measures total deviation including the manifest component and any latent phoria exposed by dissociation.
- Perform the cover-uncover test before the alternate cover test. Reversing the sequence breaks fusion first and obscures whether the deviation was a tropia or phoria.
- Interpret the movement of the uncovered eye. Do not name the deviation from the covered eye.
The Classic Trap
First, learners watch the covered eye instead of the uncovered eye. The uncovered eye makes the diagnostic refixation movement and is the one to observe. Second, performing the alternate cover test before the cover-uncover test breaks fusion and makes it impossible to determine whether the deviation was originally manifest. Sequence matters because the cover-uncover test must establish manifest status before dissociation occurs.
Clinical Pearl
The cover-uncover test is the key bedside test for separating a tropia from a phoria without specialized equipment. That distinction drives management. A tropia often warrants further evaluation and may require treatment depending on its cause, frequency, symptoms and risk of amblyopia. A phoria is generally observed unless it causes symptoms or shows poor control. Perform testing at both distance and near, since some deviations vary with fixation distance. The magnitude of any deviation found can then be quantified by neutralizing the refixation movement with prism during alternate cover testing.
