Sixth Nerve Palsy in 30 Seconds

Sixth nerve palsy causes an incomitant esotropia from weak lateral rectus function. Diplopia worsens in the field of action of the affected lateral rectus. In an older patient, think microvascular ischemia. In a child or with raised intracranial pressure signs, treat it as a neurologic warning.

What the Abducens Nerve Does

The abducens nerve innervates only the lateral rectus, so an isolated palsy typically causes an incomitant horizontal esotropia with limited abduction. Its long intracranial course makes it vulnerable to raised intracranial pressure as a false localizing sign. Compare this pattern with fourth nerve palsy, which produces a vertical and torsional deviation, and internuclear ophthalmoplegia, where adduction fails rather than abduction.

Causes by Age Group

  • Children: Benign postviral palsy can occur, but do not assume benign disease before excluding serious causes.
  • Children: Gradenigo syndrome can involve CN VI due to petrous apicitis. Otologic symptoms are an exam clue.
  • Children: Posterior fossa tumor can present with sixth nerve palsy, often through raised intracranial pressure.
  • Adults: Microvascular ischemia is the most common cause in older patients with diabetes, hypertension or other vascular risk factors. Isolated microvascular palsy typically resolves spontaneously within approximately 3 months.
  • Adults: Raised intracranial pressure can cause unilateral or bilateral palsy as a false localizing sign. Bilateral sixth nerve palsy should raise immediate concern for raised ICP.
  • Adults: Trauma and demyelination are important nonmicrovascular causes, especially in younger adults.

  • Sixth nerve palsy may be a false localizing sign of raised intracranial pressure because the nerve has a long intracranial course over the petrous apex, making it susceptible to stretch injury from downward brainstem displacement.
  • Microvascular ischemia is the most common cause in older adults with vascular risk factors and typically resolves within about 3 months.
  • Duane syndrome type 1 limits abduction, but adduction produces globe retraction and palpebral fissure narrowing. These signs are absent in sixth nerve palsy.
  • Esotropia is largest at distance and in gaze toward the paretic lateral rectus because divergence demand is greater at distance and lateral rectus weakness is maximally exposed in its field of action.

The Classic Trap

The common mistake is calling every child with esotropia and limited abduction a sixth nerve palsy. Duane syndrome type 1 is congenital and produces abduction limitation, but anomalous co-contraction during adduction causes globe retraction and narrowing of the palpebral fissure. Patients with Duane syndrome also typically have a long-standing compensatory face turn and a history of the deviation since infancy. That retraction sign and congenital onset favor Duane syndrome, not an acquired sixth nerve palsy.

Clinical Pearl

A sixth nerve palsy with papilledema does not localize the lesion to the pons. The nerve can be injured by raised intracranial pressure anywhere along its long intracranial course. Raised intracranial pressure frequently causes bilateral sixth nerve palsies, and bilateral involvement should always prompt urgent evaluation for an intracranial mass or other cause of raised ICP.
