Optokinetic Nystagmus in 30 Seconds
Optokinetic nystagmus is a reflexive eye movement elicited by viewing a repetitive moving pattern, such as an OKN drum. It combines slow tracking with rapid corrective saccades. At the bedside, it helps assess cortical visual function and may help identify disorders of cortical motion processing, including some parietal lesions.
How OKN Works
A moving target produces a slow pursuit phase that tracks the target across the visual field. The eyes then make a fast corrective saccade to refixate on a new target. Pursuit is mediated by a distributed network including the parietal and extrastriate visual cortex, dorsolateral pontine nuclei and cerebellum. The saccadic reset is mediated by the frontal eye fields.
Clinical Uses and Localizing Value
- Preserved OKN supports cortical visual function in infants or suspected malingering. Exam relevance: it provides an objective response in non-communicative or uncooperative patients.
- Reduced OKN toward the side of a hemispheric lesion, particularly with parietal dysfunction or visual neglect, is a classic neuro-ophthalmic finding. Exam relevance: this pattern classically suggests parietal cortex involvement rather than isolated occipital disease.
- Isolated occipital lesions often preserve relatively symmetric OKN despite a homonymous hemianopia, whereas parietal lesions are more likely to produce asymmetry. Exam relevance: OKN asymmetry helps distinguish parietal from occipital localization at the bedside.

The Classic Trap

The fast corrective saccade is the visible reset and defines the recorded nystagmus direction. A second trap involves direction: OKN amplitude classically decreases when the drum moves toward the side of a parietal lesion, not away from it.
Clinical Pearl
OKN asymmetry can provide additional bedside localization information beyond the visual field pattern alone. In a patient with homonymous hemianopia, reduced OKN toward the lesion side most strongly suggests parietal involvement rather than isolated occipital striate cortex disease. This distinction changes the differential diagnosis and the urgency of neuroimaging.
