Central Serous Retinopathy in 30 Seconds

Central serous retinopathy, now more precisely termed central serous chorioretinopathy (CSCR), is characterized by serous detachment of the neurosensory retina caused by choroidal hyperpermeability with secondary retinal pigment epithelium dysfunction. It typically affects men between 20 and 50 years of age, although it can occur in women and older adults, with acute painless central vision distortion. High yield for NBEO and OKAP: know the steroid link and the FA leak pattern.

How Central Serous Chorioretinopathy Develops

Increased choroidal hydrostatic pressure overwhelms the retinal pigment epithelium, producing focal RPE dysfunction that allows serous fluid to accumulate beneath the neurosensory retina. Corticosteroid exposure by any route, oral, inhaled, topical, or intra-articular, is the strongest known association, and discontinuation should be considered whenever medically appropriate in consultation with the prescribing physician. Other classic risk factors include type A personality traits, pregnancy, Cushing syndrome, and obstructive sleep apnea.

Acute vs Chronic Disease

  • Acute CSCR: single focal leak point on FA, dome shaped subretinal fluid on OCT, most cases resolve spontaneously within approximately 3 months. Exam relevance: expect a stem describing spontaneous resolution and excellent visual prognosis.
  • Chronic CSCR (diffuse retinal pigment epitheliopathy): multifocal RPE decompensation with persistent or recurrent subretinal fluid beyond approximately 3 months. Exam relevance: look for descending tract (gutter) RPE atrophy tracking inferiorly and a risk of permanent photoreceptor loss.
  • Chronic CSCR shows more widespread leakage on FA, increased choroidal thickness with pachychoroid features on enhanced depth imaging OCT, and higher recurrence risk than acute disease. Exam relevance: this distinction drives the decision to move beyond observation toward laser or photodynamic therapy.

  • Ink blot leakage is the most common fluorescein angiography pattern, smokestack leakage is classic but uncommon.
  • OCT shows optically clear subretinal fluid, often with a small pigment epithelial detachment and increased choroidal thickness with pachychoroid features on enhanced depth imaging.
  • Always ask about corticosteroid exposure by any route, even minimal or nonsystemic exposure counts.
  • Most acute CSCR resolves spontaneously with observation and risk factor modification, persistent disease beyond 3 months may warrant photodynamic therapy or focal laser in selected cases.

The Classic Trap

A common board exam pitfall is failing to ask about corticosteroid exposure in any form. Students often only screen for oral steroid pills and miss nasal sprays, inhalers, topical creams, or joint injections, all of which carry a documented association with CSCR. Dose and route do not eliminate risk, so any steroid exposure in the history should raise suspicion for CSCR in a young or middle aged patient with acute central distortion. Choroidal neovascularization should also be considered in chronic or atypical cases, particularly in older patients or when subretinal hemorrhage or exudation is present.

Clinical Pearl

Think of the RPE as a pump holding fluid out of the subretinal space. When choroidal pressure overwhelms that pump, fluid backs up under the retina exactly like a clogged drain overflowing. Laser treats focal leakage but does not correct the underlying choroidal abnormality, which is why photodynamic therapy is generally preferred for chronic disease. Fluorescein angiography is useful for identifying leakage sites, while OCT is the preferred test for diagnosis and monitoring.
