What Causes a Cotton Wool Spot

A cotton wool spot forms when a precapillary retinal arteriole becomes focally occluded, producing localized ischemia in the retinal nerve fiber layer. The ischemic insult disrupts axoplasmic transport within ganglion cell axons, causing axoplasmic material to accumulate and swell within the axons. This intra-axonal buildup, seen histologically as cytoid bodies, creates the fluffy, opaque, cloud-like appearance on fundus exam. Although cotton wool spots often resolve clinically as axoplasmic transport recovers, the affected retinal nerve fiber layer may show residual thinning on OCT, reflecting underlying ischemic injury.

Exams expect a directed systemic workup because isolated cotton wool spots may be the earliest manifestation of systemic hypertension or diabetes.

Think of a cotton wool spot as a temporary traffic jam in an axon, not a permanent roadblock. The precapillary arteriole occlusion halts axoplasmic flow, material piles up locally, and the lesion clears once flow resumes, which is why serial fundus exams and OCT can be used to track resolution over weeks rather than expecting a fixed scar. Fluorescein angiography is rarely required for diagnosing an isolated cotton wool spot but may help evaluate the underlying retinal vascular disease.
