In AMD, which drusen characteristic most strongly suggests greater risk of progression compared with normal aging changes?
Drusen in 30 Seconds
Drusen are extracellular deposits located between the retinal pigment epithelium and Bruch's membrane that anchor early and intermediate AMD assessment. For NBEO and OKAP, identify their size, margins and confluence. Large soft lesions carry greater progression risk than small hard lesions.
Types and Risk Significance
- Hard drusen: Small, discrete deposits with sharp borders. Usually lower risk when isolated.
- Soft drusen: Larger deposits with indistinct borders that may become confluent. High-risk phenotype for AMD progression.
- Drusenoid PED: Elevation of the RPE caused by confluent soft drusen. Indicates substantial drusen burden, intermediate AMD and increased risk of progression to late AMD.
4 Facts Exams Always Ask
- Small drusen measure less than 63 µm, medium drusen measure 63-124 µm and large drusen measure 125 µm or greater.
- Soft drusen are the high-risk phenotype because their indistinct borders and confluence signal greater AMD progression risk.
- AMD staging depends heavily on drusen size and type, with large soft drusen supporting intermediate AMD.
- Recommend AREDS2 supplementation for intermediate AMD in one or both eyes or advanced AMD in one eye, not early AMD alone.
The Classic Trap
Do not equate every visible druse with the same AMD risk. Hard drusen are typically small and sharply demarcated, whereas soft drusen are larger with indistinct borders. The board answer is driven by soft morphology, large size and confluence, not simply by the presence of drusen.
Clinical Pearl
Large soft confluent drusen indicate intermediate AMD. Counsel these patients about AREDS2 supplementation when criteria are met and Amsler grid monitoring for new distortion or central visual change.
