Duochrome Test in 30 Seconds

The duochrome test, also called the red-green test, is a subjective refinement step for spherical power at the end of refraction. It uses the eye's longitudinal chromatic aberration to balance the focus of red and green stimuli so the final endpoint is very close to the circle of least confusion.

The Optical Principle Behind the Test

The duochrome test uses chromatic aberration. For a given accommodative state, shorter wavelength green light focuses slightly in front of longer wavelength red light in the same eye. This fixed difference means that whichever color's focal point sits closer to the retina will look clearer, and that comparison depends only on relative clarity, not on normal color naming.

Reading the Patient's Response

  • If the eye is well corrected with the test sphere in place, letters on red and green appear equally clear. Many clinicians accept a slight residual green preference as the endpoint instead of forcing exact equality.
  • If letters on the red background look clearer, the eye's focus is still slightly in front of the retina, a residual myopic direction error. Add minus sphere, or reduce plus, to move focus back onto the retina.
  • If letters on the green background look clearer and will not balance with reasonable plus, the eye's focus is still slightly behind the retina, a residual hyperopic direction error. Add plus sphere, or reduce minus, to move focus forward onto the retina.
  • This is the classic direction rule: red clearer means add minus, green clearer means add plus. The duochrome test refines an existing spherical endpoint. It does not determine the initial refraction from scratch.

  • Classic instruction and endpoint: add plus or minus sphere in small steps until the letters on red and green backgrounds look equally clear, or the patient reports a slight preference for green.
  • Perform the duochrome test only after you have already neutralized the main refractive error with standard subjective refraction, including best sphere and cylinder.
  • The test becomes less reliable in patients with significant red-green color vision deficiency, since they may struggle to make consistent relative clarity judgments.
  • The test also becomes less reliable with advanced age or dense cataract, where reduced retinal illuminance and altered spectral transmission make small clarity differences hard to detect.

The Classic Trap

A common exam mistake is forgetting the direction rule and moving the sphere the wrong way, or treating duochrome as a standalone method to find the full refraction from scratch. The correct reasoning: red clearer means the eye still needs more minus, and green clearer means the eye still needs more plus. Reach a conventional subjective endpoint first, then use duochrome only to fine-tune the sphere.

Clinical Pearl

The accepted duochrome endpoint is not always perfect red-green equality. Many clinicians deliberately stop with a slight green preference remaining, favoring a small residual myopic bias over any hint of excess plus, since young patients can comfortably accommodate through slight undercorrection but cannot compensate for overplussing, which causes blur they cannot accommodate away.
