Lensometry in 30 Seconds

Lensometry verifies the power, axis, and prism in an existing spectacle lens. A lensmeter measures the back vertex power of a spectacle lens, allowing verification of sphere, cylinder, axis, and prism.

What the Lensmeter Measures

A lensmeter, also called a vertometer or focimeter, measures back vertex power by aligning a target image through the lens onto a movable internal system until the target comes into focus, then reading the result in diopters. The back surface rests against the lens stop, allowing the instrument to measure back vertex power. This matters with high-power lenses because vertex distance changes the effective power at the eye.

Reading a Lens on the Lensmeter

  • Center the lens over the aperture. Use the mires to locate the optical center.
  • Focus the first set of target lines. Rotate the axis wheel until both principal meridians are sharply focused.
  • Read the axis from the protractor scale and determine cylinder power from the difference between the two power readings.
  • Locate prism by noting mire displacement from the center of the reticule. Record prism in prism diopters and base direction.

  • Minus-cylinder lenses show a more minus, or less plus, reading when the second principal meridian is focused. Plus-cylinder lenses show a more plus, or less minus, reading in the second meridian.
  • Prentice's rule states that induced prism, in prism diopters, equals lens power in diopters multiplied by decentration in centimeters. Use it to estimate the prism a patient experiences when looking through a point on the lens away from its optical center, or when the lens itself is decentered relative to the pupil.
  • A manual lensmeter requires the examiner to focus each meridian, rotate the axis wheel, interpret mire movement, and determine base direction. An automated lensmeter displays these values directly after alignment.
  • Lensometry is the standard final verification that dispensed spectacles match the intended prescription before the patient leaves the office.

The Classic Trap

Do not calculate cylinder from a single power reading. Measure both principal meridians, then determine cylinder from their difference. Always center the optical center before evaluating prism, since a correctly centered lens on the mires is required before any power or prism reading is trustworthy.

Clinical Pearl

Always verify prism at the patient's intended point of wear, not simply at the geometric center. A correct prescription can still create unwanted prism if the optical centers are misplaced relative to the pupils.
