Jackson cross cylinder refines the axis and power of an astigmatic correction after an approximate sphere and cylinder are in place. It presents two rapid blur choices. The patient selects the clearer option, allowing controlled refinement of cylinder axis or cylinder power.
What the Cross Cylinder Actually Is
A Jackson cross cylinder is a lens with equal and opposite plus and minus cylinder power along two perpendicular axes, mounted so it can flip between two positions. Each flip offers an optically equivalent but oppositely oriented comparison. This makes it useful for refining cylinder axis and cylinder power after rough sphere and cylinder values are established.
Refining Axis and Power
- For axis refinement, align the cross cylinder handle with the current cylinder axis. Flip the lens and ask which position is clearer. Rotate the correcting cylinder axis toward the preferred flip position.
- For power refinement, align the handle 45 degrees from the current cylinder axis. Flip the lens and ask which position is clearer. Use the preferred response to add or subtract cylinder power.
- Make smaller axis changes as the endpoint approaches. Large changes can overshoot the correct axis and confuse the comparison.
- The endpoint occurs when the patient reports no difference, or only a trivial difference, between the two flip positions.

- Most phoropter-mounted clinical refractions use a plus or minus 0.25 D Jackson cross cylinder. A plus or minus 0.50 D version is also common, typically handheld, and used for low vision or low acuity patients where a larger step is needed to be perceptible.
- For axis refinement, align the handle with the correcting cylinder axis. For power refinement, position the handle 45 degrees from that axis.
- Perform cross cylinder refinement only after finding an approximate spherical and cylindrical correction. Large uncorrected errors degrade the comparison.
- Very low astigmatism or poor patient cooperation can produce inconsistent or unreliable cross cylinder responses.
The Classic Trap
The classic mistake is reversing the handle positions. Handle aligned with the current cylinder axis refines axis. Handle 45 degrees from that axis refines cylinder power. For axis refinement, do not rotate randomly after a preferred response. Rotate the correcting cylinder axis toward the orientation associated with the preferred flip, then repeat with smaller changes near equality.

Think "handle on axis means rotate axis." Think "handle at 45 means change power." This simple visual rule prevents the most common cross cylinder error during timed refraction questions.
