Retinal Detachment in 30 Seconds
Retinal detachment is separation of the neurosensory retina from the retinal pigment epithelium. Board questions test the mechanism first: a retinal break, traction, or subretinal fluid. Then identify foveal status and choose the management urgency.
Three Types You Must Know
Rhegmatogenous retinal detachment is the most common type. A full-thickness retinal break lets liquefied vitreous enter the subretinal space. Tractional retinal detachment results from fibrovascular membranes pulling retina away from the RPE initially without a retinal break, classically in proliferative diabetic retinopathy. Advanced disease can occasionally produce a combined tractional-rhegmatogenous detachment. Exudative, or serous, retinal detachment is subretinal fluid without a break, caused by entities such as choroidal tumors, posterior scleritis, or Vogt-Koyanagi-Harada disease. The key divider is a break: RRD needs break-directed repair, tractional disease needs treatment of traction and ischemia, and exudative disease needs treatment of its underlying cause.
Rhegmatogenous Retinal Detachment in Detail
- Classic symptoms are new floaters, flashes of light (photopsias), and a peripheral curtain or shadow.
- Determine macula-on versus macula-off status. A macula-on detachment needs same-day retinal assessment and urgent repair to preserve central vision. Superior breaks generally progress more rapidly than inferior breaks because gravity allows subretinal fluid to spread more quickly, making superior macula-on detachments especially urgent.
- Vitreous liquefaction followed by posterior vitreous detachment is the usual precipitating event. Persistent vitreoretinal adhesion can create a tear. Pigment cells in the anterior vitreous, called tobacco dust or Shafer sign, strongly suggest a retinal break until proven otherwise.
- Risk factors include myopia, lattice degeneration, prior ocular surgery especially cataract surgery, and trauma.
- Main surgical options are pneumatic retinopexy, scleral buckle, and pars plana vitrectomy, either alone or in combination. The break pattern, lens status, proliferative vitreoretinopathy, and surgeon assessment guide selection.
4 Facts Exams Always Ask
- New flashes alone still warrant urgent dilated examination. They may reflect acute vitreoretinal traction before floaters, a tear, or field loss develops.
- Macula-on RRD needs urgent assessment because fluid may extend under the fovea. Macula-off RRD remains urgent, but visual prognosis relates to foveal detachment duration and height.
- High myopia raises RRD risk because axial elongation stretches and thins peripheral retina, promotes lattice degeneration, and accelerates vitreous liquefaction.
- Tractional detachment is usually concave and tethered by membranes, often requiring vitrectomy when vision-threatening. Exudative detachment has no break, shifts with head position, and requires treatment of the inflammatory, neoplastic, or vascular cause.
The Classic Trap
Do not assume every detachment has a retinal break or needs the same surgery. Exudative detachments shift with position and have no break. Tractional detachments are concave and lack a break, though advanced proliferative disease can produce a combined tractional-rhegmatogenous detachment with a secondary break. RRD is typically mobile, convex, and bullous.
Clinical Pearl
In a patient with acute flashes and floaters, the exam question is not "Is this a posterior vitreous detachment?" It is "Did the posterior vitreous detachment create a retinal tear?" A symptomatic acute posterior vitreous detachment requires careful peripheral retinal examination including scleral depression.
